The Health Office is located on the first floor, across the hall from Room 107. One full-time registered nurse and a secretary trained in first aid staff the Health Office. Please see the Administration and Staff Directory for our current Health Office staff. Please feel free to call or stop by the Health Office at anytime with questions or concerns.
Here is some important information associated with the Health Office: - NYS requires that all new students, Kindergarten, 2nd, 4th, 7th, and 10th graders have a physical examination.
You may choose to have your child examined by your private physician or by the school physician. All health forms are due to the Health Office by October 1. Any student who has not returned their form will be scheduled to be examined by the school physician. - Students must meet New York State immunization requirements for entry into school.
- Medications can only be administered at school by following the guidelines listed below.
This includes all over-the- counter medication such as cough drops, antacids, pain relievers, etc. - Each student must have an emergency card on file which has been filled out by a parent/guardian.
If your child becomes ill or injured during school hours, we need to be able to reach you by telephone.If any of your information changes during the school year, please notify the Health Office with the new information as soon as possible. - Please notify the Health Office of any injury, contagious illness, or physical condition that your child may have so that we may make appropriate arrangements if needed.
- If your child needs to be excused from gym, a note is required.
A parent/guardian note is sufficient for a one-day excuse if the student has not seen a doctor. If your child has seen a doctor or received a doctor’s advice, a note from the doctor is required. The note must state the reason and include the length of time that your child is to be excused. All notes are to be given to the nurse at the beginning of the school day. - A doctor’s note is required for any student who requires crutches or the use of a wheelchair during the school day.
The note must include a gym excuse, the reason for the crutches and /or wheelchair and the length of time the student will need the crutches and /or wheelchair. The student must report to the Health Office upon entering the school building. - We provide First Aid only for injuries sustained at school.
We may not make medical diagnoses. - If your child would like to tryout for an interscholastic sport, he/she must pick up a permission slip in the Health office and schedule a sports physical with the school nurse.
Physicals are done approximately twice a month throughout the school year and on a first come-first serve basis.
MEDICATIONS
NYS Law as well as local regulations, strictly outline the rules that schools must follow concerning medication administered in school. Nurses are required to follow these regulations:
- The nurse should administer medication only as necessary.
- Instructions for administering of medication must be in writing from the physician and include:
a. The name of the student b. Medical condition of the child. c. The name of the medication. d. The dosage and time. e. A list of possible side effects. - A letter must be on file from the parent/guardian requesting the administration of the medication by the school.
- Medication must be brought to school by the parent/guardian, in the original bottle in which it was dispensed by the pharmacist, and NOT to be sent to school with the child.
- New prescriptions and physician’s orders are required at the beginning of each school year.
- All unused medication must be picked up by the parent/guardian, or properly discarded within seven days after it is no longer needed.
- All prescribed medications will be kept in a locked cabinet and dispensed only by authorized personnel.
- If, at anytime, the physician wishes to change the dosage, he/she must submit this request in writing.
- Special guidelines apply to field trips. Contact the school nurse for specific information.
- The term “medication” is a broad one applying to prescription and non-prescription drugs and treatments.
The Health Office provides the following services: - first-aid care to students who are injured or ill while under school supervision.
- maintenance of health records for each student.
- notifies parents of any apparent physical abnormality.
- assists doctors in the administration of health examinations.
- conducts yearly scoliosis and vision screening.
